Executive Summary
THE ORGANISATION
Streamliners focuses on the health, social care, and education sectors. We have developed a platform of content, standards, processes, methods, and software that enable us to deliver services of value to our customers, at scale, while supporting necessary local variation. We enable our customers to collaborate through networks such as the well-established HealthPathways network.
Streamliners employs over 160 staff and contractors to develop and deliver its services. The majority are based in Christchurch New Zealand, with others in Australia, United Kingdom and Canada. We are proud of our special partnerships with Yma, our delivery partner in Wales, and the internationally recognized Canterbury integrated health and social care system.
Our Purpose:
To improve the health and wellbeing of people around the world by enabling:
- community - collaborating globally for local impact
- conversations - agreeing about what will work around here
- clarity - offering clear guidance at the point of care
Our organisational purpose underlies our vision and strategy for the HealthPathways network.
This vision also underpins our other key products Hospital HealthPathways and Stronger Schools.
Hospital HealthPathways provides secondary care clinicians with access to a suite of locally agreed pathways of care within the hospital environment.
Stronger Schools is an innovative online platform that contains pathways to help school staff recognise and respond to the wellbeing needs of children and provides strengths-based, early-intervention strategies.
Our Culture:
At Streamliners, our culture is built on respect, confidence, effective communication, and personal growth. We honor the expertise and passions of our colleagues and clients, ensuring that everyone's needs and priorities are valued. Our confidence in our high standards drives us to deliver exceptional products and services. Through clear communication and partnerships, we meet and exceed customer expectations, providing value-for-money services.
Vision for the HealthPathways Network (5-10 Years)
Streamliners’ vision is that becoming a member of the HealthPathways global network is the most valuable investment that health systems can make to improve the health and well-being of their population.
Streamliners will work in partnership with the global HealthPathways network to develop pathways that:
- Save time for clinicians and patients.
- Get the latest evidence and policy into practice at scale.
- Support health equity by enabling necessary local variation of care and reducing unwarranted variation.
- Enable best use of the health system workforce and resources.
We will continuously improve the value of community membership by:
- Facilitating greater sharing of pathways, innovative models of care, and member success strategies.
- Creating opportunities to integrate HealthPathways into clinical workflows, education programmes, and system change initiatives.
- Increasing the effectiveness of our processes and tools.
To achieve our vision, we need to partner with the HealthPathways network to respond to these ten ongoing trends in health care:
- 1.Multi-morbidity - Rising prevalence of chronic conditions and multi-morbidity
- 2.Health equity - Growing priority of health equity and cultural safety, requiring greater cross-sector collaboration to address the social determinants of health
- 3.Cost pressure - Increasing cost pressures on health and social care systems
- 4.Workforce pressure - Pressure on workforce capability and capacity driving increased importance of allied health, non-regulated health workers, and care navigators
- 5.Self-management - Increasing importance of patient and whanau self-management
- 6.Changing role of general practice - Corporatisation of general practice and reducing role of general practice as care coordinator
- 7.Digitisation - Continuing digitisation of health and expectations of seamlessly integrated clinical IT workflows
- 8.Remote care - Increasing delivery of remote care (telehealth)
- 9.Personalised care - Personalised (precision) health care (big data, genomics, AI, wearable medical devices, etc.)
- 10.AI - AI-supported care delivery
